We are in search of 15 consultants who would like to support a project involving case management for a military treatment facility in New Jersey. This is an onsite project, continuing through June, with the potential for extension. Travel expenses will be reimbursed, according to contract. Details for the open positions are as follows:
Provide case management to track patients and serve as 24/7 POC for local hospitals to provide clinical instructions / discharge instructions. Additional responsibilities to include:
• Communicate to clinicians' treatment requirements, medications, etc.
• Coordinate with evacuation services for patient pick up from local hospital
• Coordinate for follow up appointments at Role 2 with staff or local hospitals and HHS for urgent care requirements
Preferred but not required:
• Healthcare operations experience managing a case management hospital department, and/or team-based projects with a focus on process re-engineering/performance improvement initiatives and broad-based change management, OR
• Case Management department experience in roles that focus Length of stay, utilization review, discharge planning, patient throughput, care coordination and interdisciplinary care collaboration
• Ability to actively manage and deliver on client expectations around project activities, timelines, deliverables, outcomes, etc.
• Ability to provide targeted communication, both verbal and written, to internal and external constituents
• Proven analytical and process redesign skills, including problem solving, quantitative reasoning, workflow process, etc.
