Please see below the link to my new Chapter of Monitoring and Evaluation made Eeeeezy (M&EZ). This chapter deals with the importance of formulating non-leading, neutral, smart and thought-through questions as part of your questionnaire. There are different types of questions for different purposes. There's also discussion on the need to constantly instruct the interviewer/enumerator on how to ask, whether to probe or not, where to skip if that's the case, and many other tips to make your survey successful (especially if your indicators are part of a "baseline" data collection!).

Hope you like it. There's so much material that I could only fit a few examples. Chapter VII-B coming up next. Feel free to share with people who might be interested.
