A discharge plan and a safe discharge are not the same thing. Every discharge carries its own risk profile, and a prior safe outcome does not rule out a fall, a missed medication, or a complication this time. Elderly patients living alone are specifically flagged in clinical research as one of the highest-risk groups for an unsafe discharge. The safer approach is to plan for the recovery window every time, rather than treat the previous outcome as a guarantee.

That’s about how long the average discharge conversation lasts, medications, warning signs, and the entire recovery plan, covered in less time than it takes to make a pot of coffee. It’s no surprise that most patients leave the hospital without fully understanding their own instructions. We pick up where that conversation leaves off. 🌧️ The Rain didn't stop us from having a great time at Caring Mission Home Care's Coffee with Veteran's today! We had a great turn out and shared a lot of new valuable information with our Veterans. Thank you to The Washington County Department of Veterans Affairs-Leigh Mawhinney, VSO, AMVETS, Bridges Hospice Veterans Volunteer Program, Marine Corp League, Operation Troop Appreciation, and especially to those who served our country. If you are a military veteran from any era you are welcome to join us. We look forward to getting together each month- no rsvp needed

August is Safe Discharge Month! It’s easy to think the hard part is over the moment someone leaves the hospital but the data says otherwise. Nearly one in five patients are readmitted within 30 days, and most people leave the hospital without fully understanding their own discharge instructions. Discharge isn’t the finish line. It’s the start of the most dangerous window in recovery, and it’s exactly where we come in. If you are caring for someone who is coming home soon, we can assist with giving you a clear picture of what the next few weeks should like!
