Here are 3 quick reminders to keep daily documentation clear, compliant, and meaningful:

1. Date Everything - Accurate dates and times help create a clear timeline of services and support audit readiness.
2. Connect Notes to the Treatment Plan - Every service note should reflect progress toward a client goal, intervention, or identified need.
3. Complete Documentation Promptly- Timely signatures and completed notes help protect staff, clients, and program integrity.

Good documentation is more than just a compliance task. Good documentation tells the story of the work your team is doing every day and helps ensure continuity of care.

The #1 Documentation Gap We See Across PRP, OMHC, and SUD Programs

If there’s one risk area that consistently shows up in audits and surveys, it is the disconnect between assessments, treatment plans, and progress notes.

Individually, each component may appear complete. However, when reviewed together, the clinical story often lacks alignment—and that is where compliance and quality concerns arise.

Common issues include:
➡ Needs identified in assessments that are not reflected in the treatment plan
➡ Treatment plan goals that are not addressed in progress notes
➡ Progress notes that describe activities rather than clinical interventions tied to individualized goals

Strong documentation should demonstrate a clear and consistent flow:
Assessment → Treatment Plan → Service Delivery → Progress Notes

Each element should build upon the previous one, creating a cohesive clinical narrative that supports:

Medical necessity
Goal-directed, individualized care
Measurable progress and outcomes
