CM Home Health Care Welcome to CM Home Health Care. Where In Home Care is given your way! All care is provided on a individual based care plan session.

Each care plan is set up for each individuals specific care needs. Services Provided:
Companion Care; Company and conversation
Basic to Extensive nursing care
Assistance with normal activities of daily living such as hygiene, meal preparation, light housekeeping, laundry, medication reminders,transportation for Appointments/Shopping, Pet Care. Assistance with mobility and some Physical/Occupation

al therapy. Personal Care:
Basic to Extensive nursing care
Bathing/Hygiene, Transfers/Repositioning, Continence/Toileting care, Special diets/Meal prep, eating assistance, oral hygiene, input & output, vital signs. Assistance with Hospice/Respite Care. Assistance with Dementia/Alzheimer's Care

Assistance with nursing education/teaching for clients & family. Care can begin with a 2 hour care session. Care sessions can be up to 12 hours per session. I will travel a 50 mile radius at a rate of 50 cents per mile. All care sessions and information collected during these sessions is protected through HIPPA guidelines and client confidentially.
