What You'll Do:
Conduct in-home supervisory visits for our adult clients and home health aides.
Perform comprehensive assessments and update care plans in collaboration with clients and families.
Manage med setups, physician orders, and care plan updates.
Assist with case openings, aide training, and respond to client needs and changes.
