The new CMS prior authorization rules were designed to make the process faster and more transparent. So why are denials still rising?

High-performing practices are taking a closer look at what happens before a claim ever reaches the payer.

Our latest blog breaks down the three reasons denials continue to rise, what practices can do to reduce their exposure, and how Pollux helps.

When it comes to outsourcing your billing, the biggest structural choice is between a large-scale billing vendor and a boutique RCM firm. Whichever direction you’re leaning, ask these 5 questions before you sign:

1️⃣ Who specifically will work my account, and will I ever talk to them directly?
2️⃣ How do you track and report denial trends by payer and root cause?
3️⃣ What does your onboarding process look like, and how do you protect cash flow during transition?
4️⃣ What experience do you have in my specialty?
5️⃣ How do you monitor payer policy changes, and how quickly do they reach my workflows?

A vendor’s answers to these questions will tell you more than any pricing sheet.

We cover more about these two choices in our latest blog post, “Boutique RCM vs. Big-Box Billing: What Specialty Practices Actually Need”.

Read it here polluxsystems.com/2026/08/28/elementor-1797/ and reach out to us if we can help you further.

More work doesn’t always mean more revenue.

When your team is spending hours chasing claims, working around inefficient processes, managing preventable denials, or manually piecing together information, adding more effort isn’t the answer.

The right systems can help your team work more efficiently, identify where revenue is getting held up, and create a more consistent path from claim to payment.

Your team shouldn’t have to work harder to keep up.

They should have the tools and processes to work smarter.

Revenue leakage doesn’t always come from one big problem. Sometimes, it’s the small gaps adding up across your revenue cycle.

At Pollux, our team helps to identify where revenue is slipping through the cracks and strengthen the processes behind it.

Find the leaks. Fix the gaps. Protect your revenue.

Tell us a little about your practice and where revenue is getting stuck. A member of our team — a real person in Evansville — will get back to you within one business day. polluxsystems.com/contact-us
