Another wonderful journey home! 💚💛

Dianne recently completed her rehabilitation stay at Vancrest of St. Marys after arriving from Joint Township, and we are excited to share that she has returned home to her family!

Throughout her stay, Dianne worked hard with the Vancrest therapy team to regain her strength and confidence, preparing her for a successful return home.

Dianne was such a pleasure to have at Vancrest of St. Marys, and now her recovery journey continues with Fountain Park Home Health!

We are grateful to be able to provide continued therapy and support in the comfort of her own home, helping Dianne build on the progress she has already made.

Earlier this week, Fountain Park Home Care had a wonderful opportunity to connect and network at the Premier Health Resource Fair for hospital case managers, social workers, and discharge planners!

At Fountain Park Home Care, we understand how important it is for patients to feel supported as they transition from the hospital back to the comfort of their own homes. Building strong relationships with our hospital partners and healthcare professionals helps us work together to make those transitions as smooth and successful as possible.

We enjoyed sharing more about our home care services, exchanging ideas, and strengthening the connections that allow us to better serve patients and their families.

We are celebrating Mary as she completes her therapy journey at Vancrest of Eaton!
