Most schedule blowouts on mission-critical projects don't start with one dramatic event.

They start quietly. A trade runs short for two weeks. Nobody catches it until the downstream crew shows up with nothing to do. The GC pushes the schedule. The next trade gets pushed. Payment applications tied to those milestones slip too.
