72% of AI decision-makers say when their organization's AI initiatives fall short, the root cause is almost always a poor data foundation.

That's the headline finding from The 2026 Hallucination Tax Report, a survey conducted by The Harris Poll on behalf of Collibra.

Nine in ten organizations are deploying autonomous AI agents right now. 87% say their teams regularly re-verify context for those agents before trusting the output, and 76% have hit critical roadblocks trying to move from pilot into production over the past year.

The pattern is consistent across every question we asked. Teams are moving fast on agents while the underlying data foundation hasn't caught up, and the gap is showing up as cost, delay and risk. Collibra helps enterprises close that gap, giving AI agents the trusted, governed data foundation they need to move from pilot to production with confidence.

Data lineage that used to take weeks now takes minutes. That's the shift Nationwide UK made to its data governance foundation.

As the data footprint for the UK's largest building society grew more complex, standards like BCBS 239 and IFRS 9 continued to require significant manual effort and point-in-time assessments.

With Collibra Data Catalog, Data Lineage and Data Quality & Observability integrated natively with Databricks, Nationwide has reduced the manual effort involved in tracking critical data and is building more automated, continuous governance controls.

Teams now know what data is available 90% faster. Data quality checks that once ran periodically now run continuously, saving roughly 75% of the time previously spent. And BCBS 239 compliance oversight is evolving from manual reviews towards more continuous and automated monitoring.
