To my 11 new Facebook family & 4 new TikTok family — Welcome to the village! I see you, I appreciate you and I thank you for following CCD Homecare’s page. I see this on a daily basis, adult children asking if we have a late appointment because they can’t take anymore time off work, our last appointment is at 4:15pm and have already been taken. They have no time to take their loved ones to get labs done, make it to their children’s events, no medication lists because, they are navigating so much and forgetting to take their of their selves. Spouses canceling appointments because they’re having surgery and are the only driver.

From Hospital to Home — We Close The Gap 💜

Hospital discharges at 11 AM. Family panics by 3 PM.
