Do you know an older person who could benefit from additional supports? They could be eligible for Aged Care Funding.

Funding is available in the following areas:
*Daily Living & Home Support
*Health & Medical Needs
*Mobility & Safety
*Memory & Wellbeing
*Social Support & Living arrangements
*Personal Preferences & Goals

To determine eligibility an Aged Care Assessment Team (ACAT) member will meet with you at your home. The person meeting with you is experienced in the field of allied health, such as a Nurse or Social Worker. Here they will talk with you about your current situation and areas that you could be further supported.

If you would like to know more about funding arrangements or how to go about booking an ACAT, or simply how to plan for later, then our Nursing staff can assist. We can schedule a time to meet with you at your home where we can help to explain the Aged Care System through to planning ahead.
