Moral injury is real and most clinicians are suffering from this daily. Be mindful that the powers that be are training you to initially care for patients, but when you hit the real world, they want you to not care. It’s conflicting.

I built a traditional practice once. I’d never do it again.

The most dangerous thing you can build is a practice that only survives because of you.

One insurance cut. One provider leaving. One health scare.

Any of those can wipe out everything you built.

I learned that the hard way. Your business shouldn’t live or die based on whether you show up.

The fix is simple: productize yourself.

Create things that generate value with or without you in the room.

That’s the difference between owning a job and owning a business.

Patients are fact-checking you now… with AI. Here’s what that actually looks like in a real session.

A home health PT had a patient pull up an AI chart that ranked every single exercise she’d ever been given, from “moderately dangerous” to “contraindicated””… despite 5 weeks of real, felt progress and zero complaints.

Patients don’t doubt clinicians who show their work. They doubt the ones who expect blind trust in a room full of tools that promise instant answers.

Every letter you can chase, I chased it.

More certifications won’t save you. They’ll just make you very good at a broken model.

Nobody questions it. You keep buying education, thinking the next cert is the answer.

Here’s the truth. You don’t have a knowledge problem. You have a packaging problem.

Learn how to communicate the information you already have.

My boss killed my illusion in one sentence.

I wanted a four-day work week. Just a little more flexibility.

He said, “”””Greg, the only way you’re ever going to get there is to move into management.””””

That was it. That was the moment.

The system doesn’t pay you more for becoming a better clinician.

There’s no billing code for years of experience. No modifier for skill. The ceiling is fixed.

Helping patients more doesn’t increase your income. Not automatically. Not ever, working for somebody else.

Once you understand the rules of the game, you stop waiting for the system to reward you.

You start building opportunities that do.

If you are looking to get out of patient care, I would recommended skill stacking as the best option. That’s where the money is at. Beats going back to school for a few years and racking up more debt.
